In a cutscene after [[One Shift More]], Paige goes to a vending machine to grab some food before she clocks out for her shift. Hailey appears, asking Paige if she can go on a trip outside the hospital. Hailey's grandmother can't leave to go visit her hometown out west, and has asked Hailey to take some photos of the desert landscape and deliver keepsakes to her relatives. Paige is unsure if she should oblige Hailey's request, but takes note of how important it sounds to her, and takes her to Ian to figure out a solution. Ian is initially hesitant to grant Hailey's request, before Paige emphasizes that this is important to Hailey, and Ian finally obliges. He then states that this trip would be a good opportunity to test out the ''Rhythm Doctor'' program's capabilities in delivering long-range treatment to patients, and sends Hailey on her way. Once the player is in the train that Hailey is traveling inside of, Ian talks to the player from the hospital and informs them of a new patient who has a sensitive heartbeat that must be taken care of via a new method of rhythm defibrillation. The player treats the Miner through his heart issues, with the Miner briefly warning the player of "the tough part" halfway through the level. After the level, the Miner and Hailey have a brief conversation where she tells him where she's headed on this trip. Then, the Miner's heart issues flare up again, as a dusty fog starts rolling in. Hailey asks the player to help them out, leading them into the next treatment.

*During this level, Hailey and Cole can be seen in their seats whenever the camera pans to the right. At release, Hailey and Cole would show different animations in response to the player missing or hitting the Miner's heartbeat.<ref>https://www.youtube.com/watch?v=VoE0YfiMarw</ref> For unknown reasons, this feature would be removed, and now they both display their idle animation regardless of the player's current performance.
*Upon release, Cole and Hailey's animations for the player hitting or missing a beat were present. Sometime later, these animations were removed for an unknown reason, and both Cole and Hailey would only display their idle animation, regardless of the player's performance. In the Act 5 update, however, these animations made a return, with both patients now responding to the player's performance again.
