About the Game
Rhythm Doctor is a 2D, pixel-art rhythm game, developed and published by 7th Beat Games and Indienova. The game entered early access on Steam and Itch.io on February 26, 2021 for Windows and MacOS. The full release came to Steam on December 6, 2025, and to Xbox on December 29, 2025. It remains updated with new additions, bug fixes, and further development.

In Rhythm Doctor, players have to accurately press their spacebar respective to a variety of different beat types and gimmicks layered over a music track within each unique level of the game. It also allows players to customize their own character sleeve with its built-in sleeve editor.

Rhythm Doctor is 7th Beat Games' second major published game, and was originally a free Flash game inspired by Rhythm Heaven, made during college summer breaks from 2011 to 2012. The first online version was released at the end of 2012.

The game received overwhelmingly positive feedback on Steam during its early access release, and has ultimately been praised for its unique story and characters, gameplay mechanics, visuals, and soundtrack.

The game includes a Level Editor, which is also available for free download without purchasing the game. It allows players to create their own levels, which they can upload to the Steam Workshop or to the non-official community Discord server, for others to play. The game supports single-player mode, shared or split screen co-op, full controller support, as well as remote play.

Ian is a character that appears in Rhythm Doctor. He is the primary coder of the Rhythm Doctor program, and he helps the player calibrate their system before handing them over to Paige to begin Samurai Techno's Tutorial. He has a focus in radiology, and appears to work better with computers than people.

Appearance
Ian has tan skin and wears grey glasses with a light blue tinge to them. He has somewhat messy black hair and is dressed in a white labcoat. Beneath the labcoat, Ian wears a grey shirt, a belt, black pants, and brown shoes. He has been described by others as having 'bloodshot eyes and delicate fingers'.

Personality
Ian is a withdrawn and relatively quiet person. Paige mentions that Ian never partied or joined any clubs in med school, which Ian begrudgingly confirms. He tends to steer towards more logical solutions and can come off as sarcastic or dry to others. Ian occasionally makes suggestions that can come off as insensitive as seen in Act 4's introduction, but he does care about others. He is a skilled gamer and has a set of old consoles that he sets up in the cafe at Nicole's persistent request. Ian, like Paige, is consistently overworked by Edega and has a tendency to work very late hours at Middlesea Hospital.

Relationships
* Ian's closest relationship is with Doctor Ada Paige. The two doctors went to med school together at Middlesea University and have known each other for years. Ian thinks fondly of Ada, and relies on her for patient care as she works easier with Middlesea Hospital's patients. He shows a great deal of concern for Ada.
* Ian is Dr. Edegas subordinate. He appears to be intimidated by Edega and does his best to meet his demands, even if they are unreasonable. He has begun to push back as seen in Unsustainable Inconsolable's tutorial, but continues to work under Edega.
* Ian is The Interns senior, and introduces them to the Rhythm Doctor program. It is implied that he has already forgotten the Intern's name in this official post, but it isn't clear. He does call the Intern pretty selfless, and compares that trait of theirs to Ada.
* Ian's Mother is not seen in-game, but Ian expresses concern for her health and stated that he had to stay with her while her carer was running late as seen in One Shift More.

Levels
History
*

Trivia
* Ian is playable in Run Ian Run, an endless runner browser game. The game is still playable and shows him in his old sprites.
* Ian uses the Expert Button, alluding to how he created the Rhythm Doctor program and knows the most about how it works.
* Ian supposedly has a Platinum rank in at least three different obscure online games.
* Ian is 27 years old, as stated in his character description.
Blurred is a level in Rhythm Doctor. It is also the first and only non-boss level of Act 7.

This level introduces Holdshots and ECG Shortening (only the former is tutorialized), and expands both Mark and Edega's story, by revealing they worked together.
|
|Good morning!
|-
|
|Hey, it took a few weeks, but I finally finished something I was working on for a while!
|-
|
|I've expanded the rhythm internship program!
|-
|
|Since we're, uh...more understaffed than ever...I figured this could help the hospital out.
|-
|
|I've started reaching out for more interns who can work remotely.
|-
|
|Rhythm Nurses...Rhythm Counselors...even Rhythm Security Guards!
|-
|
|So long as someone has a computer and an internet connection, they can take a job here. Just like you.
|-
|
|I know it's not ideal, but...ever since Ada left...
|-
|
|The few of us left here got saddled with a bunch of new responsibilities, running up and down the halls.
|-
|
|And it doesn't seem like the hospital is going to start hiring again any time soon...
|-
|
|...so this was the best solution I could come up with!
|-
|
|I've been working on the defibrillator to figure out why we haven't made any progress with Mark yet.
|-
|
|And when I'm not doing that, Edega keeps pushing me to make progress on his "Miracle Defibrillator" machine, whatever that's supposed to do.
|-
|
|I'm exhausted. On top of all that, even at home I've been staying up all night working on something that will-
|-
|
|Whatever, I'm just rambling now! You know how busy it is here. It's time to try out the new treatment on Mark!
|-
|
|Let's go check on him in the Medical Records Room before I pass out or something.
|
|Good morning!
|-
|
|We've been able to keep Mark stable and comfortable for a while, but still no progress on his condition.
|-
|
|Today might be different, though! Ian said something about a new feature for the defibrillator.
|-
|
|Hopefully this new one will be more effective. Your last treatment didn't seem to solve anything.
|-
|
|Anyway, what have you been up to?
|-
|
|...
|-
|
|Whoops. My reflex to try to make small-talk is unstoppable, apparently.
|-
|
|I'll just talk about myself, then.
|-
|
|I saw a <span style="color:#00ffff">movie</span> the other night!
|-
|
|It's been forever since I've managed to find time to do anything.
|-
|
|I wanted to go with a friend, but our schedules never line up.
|-
|
|So I went alone, but... what are the chances, I saw Logan and Hailey there!
|-
|
|It was cute. Must have been their first date!
|-
|
|Bizzare movie choice, though. It was one of those prestiege horror flicks, with all the award buzz.
|-
|
|I had to keep my eyes closed half the time. Hopefully they had more fun!
|-
|
|Anyway, let's go help Mark, shall we?
|-|Tutorial=
{|
|
|Hi! Before we start, I gotta introduce you to the new defibrillator.
|-
|
|Check it out, the Rhythm Defibrillator version 0.8.4!
|-
|
|...
|-
|
|Okay, so...yeah, it basically looks the same.
|-
|
|But feel it!!! I swapped them all out to have authentic Japanese arcade buttons!
|-
|
|They have a new glossy exterior, one millimeter of pushback travel, and are durable for up to five million lifetime presses!
|-
|
|And, uh...it's also fitted with a new treatment that will save Mr. Booker's life. That too.
|-
|
|The old defibrillator seemed like it was ineffective at treating his symptoms. It was strange.
|-
|
|So I came up with this new treatment.
|-
|
|There's still a lot of research I need to do about how rhythm treatment works, how it can be most effective...
|-
|
|...but Edega keeps getting me sidetracked with all his demands.
|-
|
|He keeps pushing me to make it into this perfect "Miracle Defibrillator." It's not very realistic.
|-
|
|He thinks we're going to be able to "rebuild cells from scratch" or something.
|-
|
|Speaking of which, you'd think he'd be here to check out the improvements, but I haven't seen him.
|-
|
|...it's probably for the best.
|-
|
|Oh! I, uh...forgot you were here. Mark, right?
|-
|
|It just means the accommodation I asked for is being honored. At least he can do <span style="color:#00FFFF">that</span> right.
|-
|
|Besides, I, uh, prefer the quiet. It's giving me a chance to catch up on my backlog.
|-
|
|Speaking of which...listen, intern. I've been reading up on this "rhythm treatment" stuff.
|-
|
|The principles of it are interesting. I think I have an idea of how you can apply it to my condition.
|-
|
|Let's test it out on myself again.
|-
|
|Uhh...usually we've got a guy for that, he's like this farmer who-
|-
|
|I'll be getting it in high doses in a minute, I might as well know what I'm getting into.
|-
|
|Now, according to the documentation, you'll see my pulse slide across the screen just like when you've performed "Supraventricular Tachycardia Beats."
|-
|
|<span style="color:#BBBBBB">(Where did he...wait, that's just my notebook!)</span>
|-
|
|As with any other "SVT Beat", you'll hear a pulse and defibrillate on the second beat.
|-
|
|Except...when you hear this special cue...
|-
|
|<span style="color:#00FFFF">Zoop!</span>
|-
|
|...you'll <span style="color:#00FFFF">hold</span> the beat instead of pressing, up until when the next 'Get' cue would be.
|-
|
|So, to be clear...you're holding the beats, not tapping them. Got it?
|-
|
|It says "If the intern doesn't remember, tell them it's like that time with the buff guy on the old-timey train." That mean anything to you?
|-
|
|Now, as much as I'd love to flip through this cover-to-cover...
|-
|-
|
|Good luck!
{|
|
|That should be everything you need to know.
|-
|
|Hey, you forgot what I wrote in the corner!
|-
|
|...the doodle of your MMO character?
|Th-The other thing!
|
|Oh right!
|-

Nicole Ting is a character that appears in Rhythm Doctor. She works at the hospital's cafe as a barista and makes her first appearance in Puff Piece. She uses exclusively Oneshot Beats and Swung Oneshots.

Appearance
Nicole has light green hair tied back in a ponytail, and light skin. Her work uniform consists of a white button-up shirt with rolled up sleeves, navy blue pants, black shoes, and an ocean-green apron layered over her shirt. She appears to be taller than most of the Rhythm Doctor cast, though she is still shorter than Miner as shown in promotional art. She is usually seen with either a cup of coffee or a cigarette in hand. Nicole is described by the janitor to usually have a sour expression on her face while working the rush hour.

Personality
Nicole is a relatively relaxed person when not on the job. She is very insightful and sympathetic to the stresses of the hospital staff. She has a generally positive outlook, making an effort to dance during her shift in Puff Piece, and loves gaming. She has a competitive side which can be seen in Bomb-Sniffing Pomeranian.

Relationships
* Nicole is seen most with Cole throughout Act 2. She admits that she finds Cole "pretty chill", and that she looks forward to seeing him every day. Nicole also has Cole's morning and afternoon usual coffees memorized.

* Following the events of Puff Piece, Nicole calls The Intern cool and invites them to come by anytime. She then tells them to make themself comfortable in Song of the Sea, offering to let them play on the keyboard if they like. She appears to be fond enough of the Intern to call them her new in-house pianist during Song of the Sea (Night), and expresses that the Intern can unwind at the cafe whenever they like.

* Nicole seems to be on relatively friendly terms with Paige, similarly calling her cool after Puff Piece. She and Paige speak at length in Song of the Sea (Night), where she expresses concern for the doctor. Nicole expresses that she finds Paige "a delight".

* In Seventh-Inning Stretch, Nicole notes to Cole that Lucky Jonronero, the new patient, is one of the biggest names on the Middlesea Scales. She is later spotted on the hospital baseball team that Lucky trains in Dreams Don't Stop, and amicably calls Lucky 'Coach', showing her respect for him.

Samurai is a recurring character in Rhythm Doctor, making his first appearance in Samurai Techno. He is the first treatable patient of the game, using solely Classic Beats at varying speeds that tend to fluctuate.

Appearance
As his name suggests, Samurai wears a full set of red samurai armor. His kabuto (helmet) adorns a golden maedate (front crest) resembling horns, and the front of his dō (cuirass) is decorated with various golden details. He also wields a steel katana with a red and purple tsuka (hilt) in his right hand. He is of short to average stature and has tanned / bronze skin.

Personality
Samurai is not a talkative character. Although he does have dialogue, it is rare for him to speak, and whenever he does, it's usually in brief, short sentences. He is known to be a frequent goer of night clubs, with an appreciation for electronic music, taking a particular liking to Cole's drum and bass music as shown in Unreachable. Of his few instances of dialogue, he is seen complimenting Cole's music several times, and thanking the two doctors in Super Battleworn Insomniac, showing a glimpse of him being amicable and kind to others.

While he's reluctant to speak, he is instead shown to be a good listener. After One Slip Too Late, Lucky recounts his rise to fame and subsequent injury to him, and when Lucky later speaks to the Miner in Seventh-Inning Stretch, he's seen overhearing their conversation, even pausing his own exercise to give it his full attention. Having learned so much about Lucky's situation, he helps him participate in baseball in a healthier manner by offering to have Lucky coach him instead, as depicted in Dreams Don't Stop. This shows a very compassionate side of him for being willing to aid a complete stranger despite having no social expectation to do so.

Unsurprisingly, he is shown to be quite fond of his armor, wearing it at all times of the day, including during strenuous physical activities. Even when playing baseball, the one time he forgoes some of his armor, he will still wear his helmet under his baseball cap.

His preferred method of locomotion appears to be rolling himself around like a boulder, as he's never shown using his legs to move.

He is depicted as a skilled skater in The 90's Decision, with the Janitor stating "he's really good" at it, although he could be referring to his rolling. He is seen skating through the Roller Disco, participating in the chase for the donut, showing a more silly side of his personality.

Relationships
*Cole and Samurai share an appreciation for electronic music. He is first seen complimenting Cole's music after 1-XN, stating he had missed Cole's music when he was away from the hospital. He is once again seen complimenting it in 2-1N, saying a brief "Good" in response to it, during the rank screen. Lastly, he is seen in 2-2N, partying with Cole and "blaring music through the hospital" with him, as stated in the pager's post-clear text. Their continued presence together, and 2-2N's post-clear pager text, suggests that the two may be friends.
*Insomniac and Samurai have an unknown relationship. The two of them share oriental theming; their levels were formerly known as Oriental Insomniac and Oriental Techno respectively, and their levels prominently feature visual oriental themes. Furthermore, Insomniac's internal name is SamuraiBoss. However, the only evidence of any direct relationship between the two characters is that they are briefly paired together during X-0, seen in Samurai's room. Thus, the nature of their relationship remains mostly unknown.
*Lucky and Samurai seem to share a bond of mutual respect, the former sharing his story with and even willing to train the latter in baseball, as well as the other hospital patients when the latter asks them to join. Samurai in turn seems quite fond of Lucky, lending an ear to his frustrations, and letting himself be wrapped in bungee cord by the Miner to serve as Lucky's training weight, without complaint. He's also seen watching a live broadcast of one of Lucky's baseball games with glee during 5-1N. Although he's not seen asking Lucky to be his friend after 5-X, one can reasonably assume they're good friends after their team's win.
*Marija, Buro, and Rin are not seen speaking much to the Samurai, but he is featured in the B-rank outcome for MD-3, where Buro gives the donut to him, much to Rin's surprise.
* The sprites for Samurai have been reworked for Rhythm Doctor's Early Access release on Steam, with his colors being notably changed.

Trivia
* Samurai is often associated with beans due to Beans Hopper.
* The game's icon features Samurai on a blue background.
* Originally SamuraiGirl was used as a second Samurai for 2 player levels, however this was later changed to Blue Samurai.
* Samurai's Pirate alt replaces him in Samurai Techno during Halloween.
|fullname=N/A
|gender=Male
}}
Appearance
The Janitor is a middle-aged black man, who wears both a surgical mask and hospital scrubs as he wanders through the hospital. He is usually seen either with his mop, or working alongside a ginger-haired janitor (such as in 5-1N).

Personality
Having worked at the hospital for five years now, The Janitor is a rather jaded and exhausted person as a whole. He commonly says that he isn't being paid enough to handle everything chaotic happening in Middlesea Hospital, but despite his words he appears rather fond of the patients and staff. He's also a rather good listener, and often has things to say about the level the player last completed.

Relationships
* The Janitor appears to at least consider The Intern friends, given how he will frequently have something to say about the level the player just completed. This is especially noted for 4-1's Janitor dialogue, where he wonders if you forgot something or just like talking to him.
* The Janitor seems a bit resigned in regards to Hailey & Logan, saying that love isn't that complicated and they should confess already and that they need to stop dancing in the plant room. Despite this, he seems a bit fond of them.
* The Janitor and Nicole are decidedly acquaintances, despite the fact that Janitor says he's been going to the cafe for five years and Nicole still hasn't gotten his name right. Despite this, he's rather fond of her music, particularly noted in 4-4N's Janitor dialogue.
* The Janitor holds Paige in high regard, especially after an incident mentioned in 4-2's Janitor dialogue where Ada handled a surgery while the patient's wife and four kids were stuck in the hospital. He's also noted when Paige gets particularly stressed, she starts drinking a lot of soda.
* For the most part, the Janitor seems to be baffled by Ian and his actions, especially his erratic schedule. He comments on how late the radiologist stays, and how he's usually the first one clocked in during 5-2N's Janitor dialogue.

Levels
* Main Ward (Not Treatable)
* SVT Ward (Not Treatable)

History
* In the Steam Demo, Janitor can be seen conversing with a coworker that reappears in 5-1N's visuals. This was meant to serve as a roadblock, much like how Janitor's cart blocked off Act 5's Physiotherapy Ward before the player unlocked it.

Trivia
* The Janitor is one of the few characters in Rhythm Doctor that the player does not operate on directly. The other characters that share this trait as of the Act 5 Update are his coworker, the Nurse, Doctor Edega, and Doctor Spider.

Doctor Gabriel Edega is a character that appears in Rhythm Doctor. He is the head doctor at Middlesea Hospital, and tends to have a cold demeanor when it comes to patient care.

Appearance
Doctor Edega has auburn hair and light skin. He carries around a dark blue clipboard that is obscuring his face at all times. He wears a set of hospital scrubs that are a greenish hue along with a pair of black shoes.

Personality
Doctor Edega appears to have a rather cutthroat approach to managing Middlesea Hospital, as he is quick to insult doctors and nurses that have quit their jobs, even going as far to accuse them of "leaving us to do all the work". He does not seem to mesh very well with the patients, let alone his subordinates Paige and Ian, who all seem to be intimidated or afraid of him. He has a tendency to make pointed comments at the performance of his subordinates, the Intern included. Edega can be spotted silently watching patients from afar, such as in The 90's Decision and Dreams Don't Stop.

Relationships
* Edega is Paiges boss, and treats her with a rather cold nature. He regularly makes comments on how she is performing, and expresses that she isn't doing enough. He has taken special note of how Paige is still struggling with swing-beats in Dreams Don't Stop, and that she constantly is distracted by other patients.
* Edega is Ians boss, and so has a tendency to push responsibilities on him regularly. He has tasked Ian with managing the Rhythm Doctor program, often pushing for him to take on even more for his workload as seen in Act 5.
* Edega is The Interns boss, and treats them with an air of indifference with the occasional note on their competency. He comments that the Intern is rather nosy for eavesdropping on Unsustainable Inconsolable's pre-tutorial conversation. If the Intern completes 5-X's tutorial with little to no mistakes, Edega praises them for their accuracy and tells them that he'll be watching their performance.
* Edega does not appear to be thought of fondly by Middlesea Hospital's patients, as Nicole comments that he "just scares me". Lucky initially went along with Edega's proposed plans, but had a turnaround at the end of Act 5 that appeared to have disappointed Edega. Ian even notes that Lucky was the most Edega has ever spoken to a patient, reinforcing that he does not directly deal with the patients himself.
Trivia
* Edega is one of the few characters in Rhythm Doctor that the player does not operate on directly. The other characters that share this trait as of the Act 5 Update are Janitor and his coworker, the Nurse, and Doctor Spider.
** The only time he ever has an ECG is in Blurred, but even then, it's automatic.
* Edega is also the only character to not show their face.
** There are brief moments in Welcome Back where Edega's eyes are depicted, either as scribbled circles or glowing red, but these are likely both stylistic choices to fit with the level's horror aesthetic.
* Before Edega's full name was revealed in Unsustainable Inconsolable, his first name was found out through musical notation that spelled G-A-B-E E-D-E-G-A.
* Edega made an appearance in the Preorder demo version of Rhythm Doctor, though it has been noted by developers that this appearance is non-canon in regards to his character.
* Edega uses the normal difficulty button.

* Ian is The The Interns senior, and introduces them to the Rhythm Doctor program. He does call the Intern pretty selfless, and compares that trait of theirs to Ada.
* Ian's Mother is not seen in-game, but Ian expresses concern for her health and stated that he had to stay with her while her carer was running late as seen in One Shift More.

Appearance
Ian has tan skin and wears glasses with a light blue tinge to them. He has somewhat messy black hair and is dressed in a white labcoat.
