Battleworn Insomniac, also known as Oriental Insomniac or 17th Century Insomniac (1-X) is a level in Rhythm Doctor. The Night Shift version is Super Battleworn Insomniac.

Battleworn Insomniac is the first boss level in the game. It introduces the Insomniac with his Insomniac Beats and Connectifia Abortus.

Synopsis
In a cutscene after Intimate, Paige and Ian explain the Rhythm Doctor Program to the player, but are first interrupted by a cocksure Dr Edega who tells them to submit a report about the Rhythm Doctor program. They are then interrupted by an emergency patient, the Insomniac. The player treats the Insomniac through glitches and disruptions caused by the Connectifia Abortus virus. After the level, Paige asks Ian about said glitches, and Ian explains that a strange virus was interrupting the intern's signal. The player moves on to the next ward.
So. We haven't explained the whole "musical doctoring" process yet.

We kinda pulled you straight into the deep end. Sorry about that.

Thankfully, you’re a quick learner.

Ian, do you think you could explain the program? It's more...your thing.

R-right! The Rhythm Doctor program is a new, experimental healthcare initiative.

It's a music-based cardiological treatment.

By listening to the patient's heartbeat and synchronising with it, we can calm the irregularities.

It's quick, and can save a lot of lives.

Plus: interns can perform the treatment remotely, there's shorter waiting times, and less work piled on the staff.

It's a win-win situation.

That's the idea, at least. As long as all goes well, we can-
Ian, did you finish setting up that remote connection?

Y-yes, sir, as you can see, our new intern is here and-
Very good.

The shift has only just started. Ensure that every patient is personally seen to.
2P Changes
* An additional color-swapped Insomniac is added to the level for Player 2, with his respective additional heartbeat row. This secondary Insomniac has the exact same heartbeat as the default Insomniac, with Player 1 taking control of the latter. Two health bars are present for both patients.

History
*
Prior to the full release of the game, this level and its super version were originally called "Oriental Insomniac". This was changed across all official sites using the song, the reason it was changed is still unclear.

Trivia
*During the first few beats of this boss level, text will be present, giving feedback on the player's inputs. The text lets the player know if they hit the beat perfectly, if they missed it by hitting too early, or if they missed it by hitting too late. This is a trait shared with 2-X, but there are two differences. Firstly, 2-X has the feedback text present for the entirety of the level, whereas 1-X has the text fade out shortly after the level begins. Second, if the player fails to press their input key and misses at least 2 beats as a result of this, text will appear on the screen which reads "PRESS ON THE SEVENTH BEAT." This text is completely unique to 1-X and cannot be triggered in 2-X or any other boss level.

*If the low health beeps are activated before the first interlude occurs, morse code will play during said interlude. The morse code spells out "MELON."

*If the virus's health bar is not depleted before the song ends, it will loop for a few more bars, fading into static, with a short segment from Samurai Techno being audible.
Invisible (Night) (4-2N) is a level in Rhythm Doctor. It is the Night Shift version of Invisible.

The track was created by bl00dwave.

Synopsis
The player simultaneously treats the Miner, Hailey, Richard Hugh and the Cockatiel through their respective heart issues, as a dusty fog rolls through during the night. The level starts with the Miner being treated, with Hailey coming in shortly after, followed up by Richard Hugh, and finally, the Cockatiel is treated independently before being combined with the other three patients, with all four being treated at the same time.

History
*Upon release, Richard Hugh's heartbeat featured solely Classic Beats. In a later update, this was changed, and Richard Hugh's heartbeat now features Row Xs. This is a feature characteristic of his heartbeats, and makes him fit better into the level.
*In the Act 5 update, this level received several changes.
::*Along with its Day Shift counterpart, this level received a visual change to more heavily include the use of fog. The background itself was also changed, now featuring a more detailed moon and a different shade of color for the night sky.
::*This level received a decrease in difficulty. In prior versions, the entire view of the level would be split into parallel screens to challenge the player's sense of rhythm and distort visual cues. This effect has been entirely removed.
::*The visual effect on the Miner, Hailey, and Richard Hugh toward the end of the level has been changed. In prior versions, the three patients' sprites would change into solid-color silhouettes. This effect has been changed to show a color flash effect on their sprites, fading in and out with a transparent element.
::*When the Cockatiel is first introduced, the Miner and Hailey will have their heartbeats join in soon after, with Richard Hugh's heartbeat being the last one to join in. A visual gag has been added for this moment; when the Miner and Hailey join in with the Cockatiel, Richard Hugh's heartbeat line will temporarily fade away, and his sprite will change to his 'miss' animation, where his face is covered with a popsicle. When Richard Hugh's heartbeat joins the other three patients, his sprite will change to show a frame of his typical idle animation, where his eyebrows are furrowed and his tail is pointing upward. This visual gag was likely added to give new comedic context behind the fact that his heartbeat is the last one to join in.
*There was a bug where Samurai Techno would begin playing at the end of the level. This has since been patched.

This is the seventh boss level in the game and the last level before the Finale. It is the first level to introduce multiple windows to the Window Dance mechanic and to introduce 7/8 time in oneshots/SVT beats. It is unlocked after beating Blurred.
I thought the Rhythm Nurses would help with running tests and checking in on all the rooms...

...but now patients keep coming to me with complaints about how the robots get stuck in the doorways.

And since they keep running in and out of the rooms to find me, it triggers the Rhythm Security Guards' sensors!
* Bitter Times is the second boss stage to be part of an act other than what it represents, after Super Battleworn Insomniac.
** However, Bitter Times is the only boss stage (or night shift of a boss stage) that is not the final level of an act, as it is presented as the second level of Act 7, while Miracle Defibrillator (Cole's Song) is the final level.
Dreams Don't Stop (5-X) is a level in Rhythm Doctor.

It is the fifth boss level in the game. It is the first time that Window Dance is used to resize the window horizontally and vertically, in conjunction with previously established window movement. It is also the first level to utilise checkpoints, having one checkpoint during the first cutscene and a health refill on the last cutscene. Finally, it marks Lucia's first appearance.

This level also introduces Blast Beats.
Synopsis
The level opens with Lucky expressing discontent over his continued state of injury, wondering how long the healing process will take. A montage then plays out, with Lucky being accompanied by the various hospital patients as time flashes by. His discontent grows into anger, leading into a conversation with the Miner where Lucky opens up about his past, worrying he will remain unaccomplished if he is unable to play baseball. The Samurai then asks Lucky if he can train him. Another montage plays out, showing Lucky training the Samurai on the field, then in a Beans Hopper cameo, as Logan and Hailey tag along to train with the Samurai. The Samurai then practices in the cafe, with Nicole pitching in to help, as Cole tags along in running with the cast. Lucky is then seeing coaching nearly all the hospital patients in the physiotherapy ward, giving a pep talk before their upcoming university game. The player then sees Ian warning Paige that the virus has returned, despite his efforts to keep it away. The view then cuts to Lucky, by himself, voicing his doubts out loud. The virus interferes as the view cuts to Lucky's dreams on the baseball field, heavily distorting the audio and visual cues. Afterwards, Lucky wakes up, in seemingly brighter spirits, as the view cuts to a representation of the hospital patients moving around on a heart-shaped baseball field, zooming in to focus on the Samurai using his bat to swing at baseballs. Lucky is then seen, in his typical baseball outfit, giving another pep talk to the hospital patients. His sprite gradually fades into his present-day self, with his arm cast and lack of baseball material, telling the patients that they will "live with no regrets" if they continue to push themselves. The lyrics fade in again during this moment, as the resolution of the level plays out. A dusty grain effect can be seen after another baseball field montage, displaying Lucky's past memories of him serving on the baseball field, of him standing on the hospital balcony, and of him engaging in exercises with the Miner. The level's final beat ends on the Samurai swinging at a baseball. After the level, Paige checks up on Lucky, to which he claims that he is feeling alright. Edega then speaks to Lucky about the aforementioned new rhythm treatment, now ready for use on patients, with the possibility of healing him permanently and in a fast manner. Lucky declines, stating that the friendly company of the hospital patients has made him want to stay at Middlesea Hospital, and that he has thoroughly enjoyed coaching them. Additionally, he cites the Miner's line about having "only one body," and that he does not want to risk permanently damaging it via risky treatment methods. Edega attempts to further pressure Lucky by claiming his contract is on the brink of being canceled and that he is disappointing his baseball fans. Lucky persists in declining the offer, claiming he has "found a better team." Ian then tells Paige that the cause of the virus has been the rhythm defibrillator itself all along. He claims he can fix it, but that he needs more time to do so. He says he'll need her and the intern to distract Edega in the meantime. Lucky then apologizes to the two doctors for his previous insults, before challenging the intern to an arm wrestle. The Miner, Nicole, and Hailey then compliment Lucky for his new friendly attitude, before Lucky tells them to shut up, out of embarrassment. Paige then tells the hospital patients to rest up, as Lucky chimes in to tell them that they'll be jogging laps around the block tomorrow. Paige then tells the intern that they have more work to do; the dialogue then transitions into the credits level, Helping Hands.
Hello.
Dr Paige was supposed to be here to assist you, but she seems to be running late.
Getting distracted by other patients, presumably.
Before your next procedure, there's a few things you need to be prepped for.
<small>[Tutorial Begins]</small>
You've done an excellent job so far, regulating Mr Jonronero's heartbeat during this difficult time.
It's still behaving erratically, so there's a few new patterns to be prepared for.
According to Ian's notes, this first exercise is called the <span style="color:#ff0000">blast beat.</span>
It follows those "Get, Set!" cues that you learned back in the SVT Ward...
...but it's a bit off-time from how it usually goes.
Watch this demonstration.
As you can hear, there's an extra whistle cue from the nurse to signal this type of beat.
Our patients' heartbeats are always unpredictable.
If someone is going to master this defibrillation system, they need to be ready for anything.
Try not to miss any beats, even if that quick transition catches you off-guard.
Let's see how you do.
<small>[Player practices Blast Beats.]</small>
Good enough.
There's another pattern you should brush up on.
Mr Jonronero's heart is subdivided strangely. And sometimes it beats in quick chunks.
I've told the nurse to warn you with a cue when this appears. She'll shout "One!"
One last test...
<small>[If the player has no misses:]</small>
Excellent. You never miss a beat, do you?
Last time I saw Paige using the rhythm defibrillator, she was still struggling with swing beats.
Hm...
Ian is lagging behind on his research. I need you to keep the patient stabilized until he's done.
It may take a few weeks, at this rate.
Once the new defibrillator is finally ready, we'll have a landmark procedure on our hands.
You've done a lot for us in a short time. A promotion would be well-earned.
I'll be watching.

But on the field, everything was different.
Without baseball... I feel like I'll just be that useless kid again.
Ninth inning, team!
I know how it feels. Like you're running on an empty tank. That it'd be easier to just let it be over.
But don't give in.
Y'know, when you all said you wanted me to coach you...
That's why I've learned never to listen to what coaches have to say. They're usually clueless.
So listen to me just this once, then never listen to me again!
Turns out that was exactly what she needed! Using those old ratty things, she sewed together those really cute baseball uniforms.
I figured maybe my energy would be better spent somewhere else.
To all my fans (To all the fans)
I tried my best but things are tough
I'm out of luck (You're out of luck)
My joints are torn, I can't hold on
Everybody believes there must be
Something wrong with me,
To stay this way
The air is still
But there's no peace inside this place
Fans lose their cool
My team moves on, I've been replaced
I'm fed up with waiting
My body is fading
I'm sick of your sympathy

[Outro]
I'm stepping away
From all my regrets of yesterday
I'm more than my dreams
It's time to move on
To all of my friends, I'm holding on
Trivia
* This level now marks Lucia's first appearance within a main story level. Prior to the Act 5 update, Art Exercise was the first and only level Lucia could be seen in, as a patient or character. This makes it one of the few Early Access levels to retroactively change the first appearance of a previously established character.
* This is the only boss stage to have a tutorial.
Intimate (Night) (1-2) is a level in Rhythm Doctor. It is the Night Shift version of Intimate.

Synopsis
The player treats Hailey as she expresses confusion and sadness over Logan's reluctance to confess his romantic feelings to her. Halfway through the level, Logan insists on being treated in the same room as Hailey, attempting to build up the courage to confess his feelings to her. Unlike the day level, where the player had Paige's cooperation to treat the two patients, Ian does not cooperate with the player to treat both patients at once. Since his game "finally found a match," he leaves the player to treat both patients at once by themself. After the level, Hailey tries to get Logan to confess his feelings, but he is once again too nervous to properly do so, and leaves abruptly, leaving Hailey confused and sadder. The post-clear text for Hailey has her saying "Everything will be okay. I think he's just shy," signifying that Hailey is feeling better despite the lack of resolution.
2P Changes
* After the first four inputs, Logan appears in his ward, controlled by Player 2 and featuring the same heartbeat as Hailey before the first interlude. After the first interlude, the Xs on Logan's row disappear as a new pattern of beats emerges, coinciding with an added piano melody. The Xs reappear as the piano melody fades away before the last four inputs.

History
* In the Flash demo, Logan remains as the initial patient, with a "Samurai Girl", nearly identical to Samurai in appearance except for lighter facial skin representing make up, appearing instead of Hailey.
** The doctor says they're going on a night out, with Logan reacting to the new patient in bewilderment:
::{|

"What..?"

".."

"Who on earth is this?!"
* Before the 2P Co-Op update, Player 1 had full control of Hailey's standalone heartbeat, with Player 2 having no gameplay input until after the first interlude, where they take care of the second patient's heartbeat. Thus 1-2N has less input complexity than 1-2 as the two players had to juggle control of the first patient's heartbeat.
* The 2P chart received an overhaul upon the release of the 2P Co-Op update.

Trivia
*In-game, this level lists Hailey as being the sole patient. This is untrue, as Logan is another patient the player has to treat in the level. This is likely done on purpose; the Day Shift counterpart has the player only treating a single patient, which sets up the surprise for the Night Shift level to have the player treating both Hailey and Logan at once.
*Ian's dialogue mentions his game having "found a match," leaving the player to take care of both Hailey and Logan. This contrasts 1-2, where Paige cooperates with the player and takes care of the second patient. However, in 2P mode, 1-2 has altered dialogue where Paige simply instructs Player 2 to take care of Hailey. Since 1-2N has no altered dialogue for if 2P is enabled, this unintentionally makes Ian's dialogue out of place; Paige has demonstrated that it is valid to instruct the second player to take care of a patient, so Ian has no need to bring up the fact that he needs to attend his game, and he should instead instruct Player 2 to take care of Logan.
Intimate (1-2) is a level in Rhythm Doctor. The Night Shift version is Intimate (Night).

It introduces the characters Logan and Hailey, and the Row Xs mechanic.

Synopsis
The player treats Logan for what Ian calls "excessive schmaltziness." Halfway through the level, Hailey insists on being treated in the same room as Logan. The player and Dr Paige cooperate to treat the two, Paige handling Hailey's row. After the level, Hailey explains to Logan that she got worried since he had told her he was going to the hospital without any explanation. Logan apologizes, explaining that he said "something stupid" in their last conversation, before Hailey cuts him off to reassure him, saying that they now have time to discuss what it was that he wanted to say. Paige then continues the player's tour of the hospital.
</tabber>

2P Changes
* The tutorial for this level introduces the "Switch!" cue, where control of a row is passed from one player to another.
* Player 1 and Player 2 swap control of Logan's heartbeat three times. After the first interlude, Player 2 takes control of Hailey's heartbeat instead of the CPU. Paige's dialogue changes, directing Player 2 to take care of Hailey, while Player 1 takes control of Logan's heartbeat. The two players retain control of their respective heartbeats until the level ends.

History
* There were formerly two instances of inputs being two beats apart on Logan's row. Following a nerf, the second input has been removed in each of these instances, although they remain in 1-2N.
* Before the 2P Co-Op update, there was no tutorial for rows switching between players, and the switches had no audible cue. "Switch!" cues were added in the 2P Co-Op update alongside their tutorial.

Trivia
* Since 1-2 and 1-2N have identical beat patterns, with the only difference being a change in dialogue and visuals, playing in 2P mode makes it possible to functionally experience 1-2N before 1-2N is actually unlocked.
* This is one of two levels in which 2P mode enables player control of a patient whose heartbeat is normally controlled by an NPC, the other being Distant Duet.

I've been a fan of Rhythm Doctor for a quite a while, no doubt, though I actually only started playing the game in 2026. Back on the Mario Wiki, I was responsible for adding a lot of the 2-player variants to the microgames in WarioWare: Get It Together!. No idea why, but I'm weirdly obsessed with the difficulty levels of microgames. And some of the 2-player variants are kind of like extra levels, so...

...you bet this is definitely analogous to night shifts and the 2P versions of levels. Not sure what I'll contribute to here, but hey, you never know what'll work out unless you try.

<center><big>Some fun facts on me:</big></center>

* My gateway into Rhythm Doctor was through... this old Lockstep level.
** What followed after is GDColon's recreation of Rhythm Doctor in Geometry Dash, which seemed to be a motivator for many as well.
** And then... Delay Pedal got involved in it somehow? Maybe because it's a scrapped level, but either way, always had my eyes out for it.
